چکیده

A 23-year-old woman was admitted for brief, recurrent spells of intense fear and visual hallucinations. During video-EEG study, one of the spells showed no definite ictal EEG changes on longitudinal bipolar montage for 26 seconds while evolving rhythmic discharges were readily visible on Cz reference. Because the pattern was evenly distributed in the temporal region at onset (figure), it “canceled out” in the bipolar montage. It is conceivable that a shorter seizure would have been missed if reviewed only in bipolar montage. Hence, this case is a reminder of why EEG should be reviewed in bipolar and referential montages before concluding that a spell has no EEG correlate.

An 11-year-old, right-handed girl was admitted for video-EEG monitoring of nocturnal paroxysms consisting of eye opening, groans, right arm shaking, and altered consciousness (see video on the Neurology Web site at www.neurology.org). Ictal video-EEG suggested a left frontal seizure; subsequent behavioral disturbances including dysrhythmic right arm shaking and confusion continued without elect...

Alpha Wavelet Power as a Biomarker of Antidepressant Treatment Response in Bipolar Depression.

There is mounting evidence of a link between the properties of electroencephalograms (EEGs) of depressive patients and the outcome of pharmacotherapy. The goal of this study was to develop an EEG biomarker of antidepressant treatment response which would require only a single EEG measurement. We recorded resting 21-channel EEG in 17 in-patients suffering from bipolar depression in eyes-closed a...
